Online COVID-19 Screening Tool Available For Macomb County

Mt. Clemens, MI - Macomb County has launched a daily self-screening questionnaire for employees, vendors and members of the public wishing to enter county facilities.

The digital survey will ask for a name and contact information. It will also list COVID-19 symptoms and will ask respondents to check off what they may or may not be experiencing. Immediate guidance on whether the respondent should enter a county building will be issued based on their reported symptoms. Information gathered from the survey will be kept confidential.

The self-screening questionnaire was developed to assist county leaders in keeping facilities safe and secure and in accordance with CDC guidelines and the Governor's Executive Orders. The goal is to prevent individuals with COVID-19 symptoms or a positive COVID-19 test from entering county buildings. County leaders also hope to educate the public about symptoms and safety protocols in dealing with the novel coronavirus.
